錐体路（皮質脊髄路）

機能 / Function

一次運動野(M1)などから起こり、放線冠→内包後脚→大脳脚→橋→延髄錐体を下行する。大部分の線維は延髄下端の錐体交叉で対側へ交叉し、脊髄側索を外側皮質脊髄路として下行し、前角の運動ニューロンおよび介在ニューロンへ投射する（一部は単シナプス性に運動ニューロンへ入力）。随意運動の実行指令を伝える主要な下行路。

Arises mainly from the primary motor cortex (M1); descends through the corona radiata, posterior limb of the internal capsule, cerebral peduncle, pons and medullary pyramid. Most fibers decussate at the cervicomedullary junction and descend in the contralateral lateral corticospinal tract to anterior horn motor neurons and interneurons (some fibers synapse monosynaptically onto motor neurons). It carries the command for voluntary movement.

回路破綻による症状 / Symptoms of disruption

障害で対側の上位運動ニューロン徴候（痙性麻痺、深部腱反射亢進、Babinski 徴候陽性、病的反射）。内包後脚の小病変では対側の純粋運動性片麻痺を来す。錐体交叉より上の病変は対側、脊髄（交叉より下）の病変は同側の麻痺となる。

Lesions cause contralateral upper motor neuron signs (spastic paresis, hyperreflexia, Babinski sign). A small lesion in the posterior limb of the internal capsule can cause contralateral pure motor hemiparesis. Lesions above the decussation → contralateral weakness; spinal lesions below it → ipsilateral weakness.

皮質脊髄路を選択的に侵すのは、運動ニューロン疾患(筋萎縮性側索硬化症 ALS・原発性側索硬化症の上位運動ニューロン障害)、遺伝性痙性対麻痺(軸索の長さ依存性の変性)、脱髄疾患(多発性硬化症)である。血管障害(梗塞・出血)は内包・放線冠・大脳脚・橋など走行のいずれの部位でも対側の上位運動ニューロン徴候を生じる。

The corticospinal tract is selectively affected in motor neuron disorders (upper-motor-neuron involvement of amyotrophic lateral sclerosis [ALS] and primary lateral sclerosis), hereditary spastic paraplegia (length-dependent axonal degeneration) and demyelinating disease (multiple sclerosis). Vascular lesions (infarction or hemorrhage) anywhere along its course — internal capsule, corona radiata, cerebral peduncle, pons — produce contralateral upper-motor-neuron signs.
