内側前頭前野・後帯状皮質/楔前部・下頭頂小葉(角回)・外側側頭を含み、外的課題が無い安静時に活動が高い。自己参照的思考、エピソード記憶の想起、心の理論(社会的認知)、将来の展望(prospection)、内的思考に関与する。

Includes the medial prefrontal cortex, posterior cingulate/precuneus, inferior parietal lobule (angular gyrus) and lateral temporal cortex; most active at rest. It underlies self-referential thought, episodic memory retrieval, theory of mind, prospection and internal mentation.

ネットワーク破綻による症状 / Symptoms of disruption

アルツハイマー病で早期から後帯状/楔前部・内側側頭の機能と結合が低下し、エピソード記憶障害と関連する。自己参照・社会的認知・将来展望の障害。後帯状/楔前部の障害は記憶・意識水準にも影響し、意識障害との関連も示唆される。

In Alzheimer's disease, the posterior cingulate/precuneus and medial temporal connectivity decline early, correlating with episodic memory loss. Impaired self-referential processing, social cognition and prospection; PCC/precuneus damage also affects memory and level of consciousness.

臨床症候・疾患 / Clinical correlations

アルツハイマー病で後帯状皮質/楔前部・内側側頭の結合が早期から低下し、エピソード記憶障害と相関する(アミロイド/タウ画像とも関連)。後帯状/楔前部の急性病変は記憶・意識水準にも影響しうる。

In Alzheimer's disease, posterior cingulate/precuneus and medial-temporal connectivity decline early and correlate with episodic memory loss (and with amyloid/tau imaging). Acute PCC/precuneus lesions can also affect memory and level of consciousness.

Clinical pearl — AD では後帯状/楔前部の機能・結合低下が早期マーカー。エピソード記憶障害と相関。

PCC/precuneus connectivity decline is an early AD marker, correlating with episodic memory loss.
